1. Download the Forms

To provide you with personalised support, we first need a few details from you. Please start by downloading and completing two forms:

• 📄 [Onboarding Form and Consent Form] – This helps us understand your needs and background, gives us permission to work on your behalf.
• 📄 [Terms and Conditions document] – Please read these and ensure that you understand the terms and conditions under which we provide our services. We would be happy to clarify any item that may be unclear.

You can print and fill them out by hand or complete them electronically if that’s easier for you.

2. Fill In Your Details

Take your time to fill in both forms carefully. This information helps us advocate for you more effectively — whether it’s with medical schemes, hospitals, or doctors.

• Make sure your contact information is correct
• Don’t forget to sign the consent form!

3. Send the Forms Back to Us

Once completed, you can return the forms through whichever method works best for you:

4. We’ll Contact You

After we receive your forms, one of our case managers will reach out within 24 to 48 hours. They’ll:

• Start working with you on access to treatment, and care coordination
